Tone: clear, calm, and reassuring where appropriate. 
Do not minimise progression if it has occurred. Maximum 300 words.

Why this works

The explicit instruction about symptoms to prompt early contact is particularly important in valve disease, where the transition from asymptomatic to symptomatic is the key clinical turning point. Making this clear in the letter empowers the patient to be an active participant in their own monitoring.

How to tweak it

  • If the patient has been referred for surgical or transcatheter valve intervention, adjust the plan section and add: "Include a paragraph explaining why referral for valve intervention is being considered and what this process involves next."
  • To produce a letter specifically addressing a patient's concern about flying or exercise, add: "The patient has asked about [[specific_concern — e.g. 'whether they can continue long-haul flying']]. Include a brief, accurate response to this."
